Collaboration

Improving health care equity requires collaboration between a certified organization and its community, including the patients, families, and caregivers it serves. The patient population of each organization is unique. In order to improve health care equity, an organization has to understand and meet the unique needs of its patients and community, and this requires community engagement.

A collection of curated resources have been identified to help organizations meet the certification requirements. Resources can be filtered by element of performance (EP), which have been labeled to indicate the focus of the requirement.
