Quality compounding of medicine is foundational to patient safety and may serve to reduce the incidence of adverse healthcare associated conditions such as infections and medication errors. USP Compounding Standards <797> and <795> aim to assist hospitals in improving the quality of the healthcare they deliver. About the Program

The Joint Commission’s Medication Compounding Certification is a voluntary certification program for hospitals and home care agencies seeking an independent evaluation and recognition of compliance with USP Chapters <797> and <795>.  Michigan is the first state to require certification, however several other states have legislation pending.  The Joint Commission will be certifying over 75 organizations in Michigan in early 2017.

The program has been developed to recognize organizations that demonstrate excellence in medication compounding through a combination of standards compliance for both Joint Commission standards for medication compounding and the USP standards.

Focus Topics

  • People:  training, proper use of personal protective equipment, aseptic technique
  • Product:  sterility of base products, beyond use dates, labeling
  • Environment:  airflow, buffer areas, guidelines for cleaning and documentation, storage
     

The Risks of Compounding

Pew’s Drug Safety Project identified over 25 reported compounding errors or potential errors associated with 1,076 adverse events, including 90 deaths, since 2001.

Eligibility

Compounding pharmacies that provide sterile and nonsterile compounding services are eligible to apply for certification. An earned certification is good for two years. Unlike other Joint Commission certification programs, you do not need to be a Joint Commission-accredited organization to gain this certification.

This program is open to all organizations performing medication compounding.

How to Get Started


Step 1.  Get a 90 day trial of the standards
The Standards of this certification program are aligned with the USP standards. Meeting these standards can help show your patients that you are providing quality compounded medications.

Step 2. Conduct a gap analysis. 
Whether you have internal resources to conduct an assessment, or are looking for an external objective view, this is an important step in gauging compliance and priorities.
