Disease-Specific Care Certification is an option for nursing care centers seeking to demonstrate the quality and safety of their organized, programmatic approach to continuing care, treatment, and services for patients and residents with specific diagnoses or post-procedure needs (for example, rehabilitation following orthopedic or cardiac procedures or injury, stroke diagnoses, trauma related injuries).

Integrated Care Certification
Integrated Care Certification focuses on assessing how well clinical care partners, such as physician practices, ambulatory clinics, hospitals and nursing care centers, communicate, collaborate and coordinate with one another to provide patient-centered, clinically integrated care, treatment, and services to their patient population. The certification evaluates planning for clinical integration, including information sharing/exchange, patient handoffs, care planning, and performance measurement and improvement that spans the continuum of the clinical partners’ care, treatment, and service offerings.
