For both waived and non-waived testing, two criteria must be met when reference ranges are not supplied on the same page along with the laboratory result: The reference range must be located elsewhere within the permanent medical/clinical record. The result must have a notation directing the reader to the location of the reference range(s) in the medical/clinical record.
