Health Care Equity - Health-related Social Needs (HRSNs)
Our hospital has several clinics that are surveyed under the HAP manual. If we collect a patient's health-related social needs (HRSNs) in the clinic, do we need to collect the information again upon admission to the hospital?
Any examples are for illustrative purposes only.
The hospital determines the patient population that it will assess for HRSNs and which HRSNs to include in the assessment. If the HRSN data for the patient population defined by the hospital was collected in a hospital-affiliated clinic, it does not need to be collected again if it is available upon admission to the hospital. It is recommended that data collected during a previous encounter (outpatient visit, previous admission) should be reviewed for accuracy during hospital encounters.
