Medication Inclusion Exclusion - IV Thrombolytic Therapy
How can the organization indicate IV Thrombolytic Therapy has been considered for a stroke patient?

If the ischemic stroke patient arrives to the hospital ED within 2 hours of symptom onset (i.e., Time Last Known Well)) and does not receive IV t-PA within 3 hours, then the reason for non-treatment (i.e., Reason for Not Initiating IV Thrombolytic) must be documented by the MD/APN/PA or pharmacist on the day of or day after hospital arrival in order to exclude the case from the STK-4 measure population. The organization determines where this information is documented.
