This Sentinel Event Alert has been retired. Current guidance is provided in the Institute for Safe Medication Practices’ (ISMP) September 2013 advisory, and its 2016-2107 Targeted Medication Safety Best Practices for Hospitals. Also see the World Health Organization’s July 2007 alert, which endorses the practice of using minibags instead of syringes for dispensing vincrinstine.
