Sentinel Event Alert 23: Medication errors related to potentially dangerous abbreviations

This Sentinel Event Alert has been retired as of June 2016. This topic is addressed in Information Management standard IM.02.02.01, elements of performance 2 and 3, which include a “Do Not Use” list of abbreviations. Also, the Institute for Safe Medication Practices (ISMP) has a List of Error-Prone Abbreviations, Symbols and Dose Designations, including those used in electronic medical record (EMR) systems.
