The Joint Commission, in partnership with Kaiser Permanente, seeks to recognizes a healthcare organization that directly delivers healthcare and its partners that led initiatives that achieved a measurable, sustained reduction in one or more healthcare disparities. The award offers an organization the opportunity to earn national recognition for its efforts.

An organization wishing to apply must meet the eligibility requirements.
