The Joint Commission’s Disease-Specific Care Certification Program, launched in 2002, is designed to evaluate clinical programs across the continuum of care. Joint Commission accredited organizations may seek certification for virtually any chronic disease or condition.
Disease-Specific Care Certification is available at two levels, core and advanced. Both are evaluated through the same set of Disease-Specific Care standards. However, the advanced certification programs must meet additional, clinically specific requirements and expectations.
