The Joint Commission is hosting a series of 90-minute conference calls with health care practitioners directly involved in care transitions from various health care settings to gain insight into their challenges. For these conversations, The Joint Commission is reaching out to front line staff – discharge nurses, care coordinators, case managers, social workers and others – to share their experiences. Long Term Care: Wednesday, June 13, 10-11:30 p.m. CT
Home Care: Wednesday, June 13, 1:30-3 p.m. CT
Hospital: Thursday, June 14, 10-11:30 a.m. CT
Behavioral Health: Friday, June 15, 10-11:30 a.m. CT
Ambulatory Care: Monday, June 18, 1:30-3 p.m. CT
Participants must represent a Joint Commission-accredited organization. If you are interested in participating, sign-up by Friday, May 18. Each focus group is limited to 15 participants. Final candidates will be notified by e-mail. The Joint Commission enterprise supports patient safety across all settings through accreditation and certification requirements, National Patient Safety Goals, performance measures, survey activities and educational services. We are working to ensure that these methods remain relevant, contemporary, and comprehensive in addressing the challenges associated with transitions of care, with the goal of improved patient safety and quality of care.
