Quality Check Help - Beginning Your Quality Check Search

Welcome to Quality Check.  You can search for a health care organizations in three ways, by organization name, by zip code or by state and city. 

1. Enter an organization name (select a state to limit the search results)
   or
2. Enter a zip code (select a distance from zip code to widen the search)
   or 
3. Select a state from the drop down list (then select a city to limit the search)
