Disease-Specific Care

Consumer Brochure:  Assessing the Quality of Care in Disease Management

Choosing to be certified by the Joint Commission means a great deal.  It means you've demonstrated a commitment to providing quality care and you seek to continuously improve that care.  We feel it is important to educate your current and prospective patients on the importance of your Joint Commission certification and what it means to them.  That is why we created this brochure.

Download Instructions

Due to its size we recommend the Macintosh file be downloaded by a professional printer only.

These are compressed folders containing Macintosh-format files and fonts. They also contain a "Read Me" for a commercial printer with all specifications (i.e. ink colors, paper stock, etc.)  Do not try to decompress the folder if you are not on a Macintosh system. A windows version is not available.

No changes in, additions to, or deletions to the text should be made without prior written approval from the Joint Commission. You may personalize the brochure by adding your logo and information on the blank space on the back panel of the brochure.

If you have questions concerning these files, contact drutkowski@jointcommission.org.

To the printer:

  • These folders contain all the elements needed to produce The Joint Commission: Assessing the Quality brochure.
  • A Macintosh-format Quark 7.31 file, all fonts and support graphics are included.

The following are the basic specs:

  • size-flat 8.5"x11", folds twice to 8.5"x3.66", creating a trifold brochure
  • 4 color process both sides, bleed
(Note: On the Joint Commission version of this brochure, an overall aqueous coating was added for extra protection during distribution. This is not necessary but may be considered if budget allows.)
  • The back (page 6) is blank and available for personalization.
  • No trapping has been done to this file.

The Macintosh file was professionally created by our design staff. This file is print industry standard, four color process.
