Authentication Using A Single Document

Q: Can a single document be used to authenticate all unsigned orders, notes, and so forth in a medical/clinical record?
 
A: Yes, under limited circumstances! A single statement signed by the practitioner indicating that all unsigned notes, orders and other entries are hereby authenticated, without a listing of every item covered, would not meet this standard. However, it would meet the standard if the single document listed all the specific items to be authenticated and there was an opportunity as well for the practitioner to authenticate each entry on the document itself if they so chose. The use of such a "global" authentication process is guided by medical/clinical record policies and procedures.
 
In addition in the CAMLAB the standards require that pathology and laboratory reports requiring specific interpretation have to be authenticated by the individual making the interpretation.
