H&P Older Than 30 Days/Using the Prenatal Record

Q: Can a history and physical which is older than 30 days be utilized, provided it is updated to reflect the patient's status at the time of admission/service?

A: No. Standards stipulate that an H and P which is performed up to or no more than 30 days before admission may be utilized provided it is updated to reflect the patients status at the time of admission/service.

It is recognized that the prenatal patient is a special situation in that, in and of itself, the prenatal course of care is a planned, systematic updating of the history and physical performed at the first visit and throughout the pregnancy. As such, the entire prenatal record can be utilized as the history and physical, provided it is updated to reflect the patient's condition upon admission.
