What's the process to become accredited?

  1. Read our online informational toolkit.  Learn:
    • Services we accredit
    • Patient volumes required/general eligibility
    • Information on our Medicare certification/deemed status survey option
    • Other helpful information!

  2. Request access to our online application
    • You'll receive an email within 3-5 business days with a password to access the online application at our home page
    • Choose a “ready month” for your initial survey on the application within the next 12 months that allows you time to prepare.
    • View a brief demo on how to access your online application. Questions? Call your Account Representative at 630.792.3007.

  3. Obtain and review our accreditation requirements. 
     
    Once you submit your application for survey you will receive a free copy of the standards manual (both a print version and electronic version) once your application deposit is processed.

  4. Pay your application deposit online once your application is submitted.
    Applications must be accompanied by a nonrefundable $1700 deposit to be processed.
