Suggested Office-Based Surgery "Ready to Go" List

It would facilitate the survey process if the following items could be readily available to the surveyor on the date of the survey:

  • Evidence of compliance with the Joint Commission Public
  • Job descriptions for non-LIPs.
  • Billing list by provider for last four months (procedure/CPT code).
  • Surgery or procedure log for last four months (if available).
  • Contracts for all direct services provided.
  • Any written policies or procedures used in the practice.
  • Any performance improvement information such as data, trended information and any other documents supporting a performance improvement process in your practice.
  • Practice guidelines/clinical protocols.
