It would facilitate the survey process if the following items could be readily available to the surveyor on the date of the survey:
- Evidence of compliance with the Joint Commission Public
- Job descriptions for non-LIPs.
- Billing list by provider for last four months (procedure/CPT code).
- Surgery or procedure log for last four months (if available).
- Contracts for all direct services provided.
- Any written policies or procedures used in the practice.
- Any performance improvement information such as data, trended information and any other documents supporting a performance improvement process in your practice.
- Practice guidelines/clinical protocols.
